Everything that was in vanilla Emerald is still where it was. This is the list of changes and additions — the items that moved, the items that are new, and the NPCs that hand them out.

Oldale Town: the "cheating girl"

Next to the PokéMart, a girl gives you four things at the start of the game: a Healing Heart, an Infinite Repel, a Rare Candy Box, and the IV/EV editor built into the summary screen. If you want to play without them, the challenge options can restrict what you're allowed to use.

Nature mints

Mints change a Pokémon's nature. They arrive after the fourth gym if you enabled them at the start of the game, or after you become Champion if you did not. The full set is at the Flower Shop, and four are found in the world:

Modest Mint
Lilycove City
Timid Mint
Mauville City
Rash Mint
Meteor Falls
Calm Mint
Route 111 sand tower — miss it and it is gone for good

The four shards and the Kanto fossils

Shards come from Rock Smash drops and from specific wild Pokémon:

Green Shard
Wailmer, Wailord, Relicanth
Yellow Shard
Chinchou, Lanturn (rare)
Red Shard
Corsola, Carvanha, Sharpedo
Blue Shard
Tentacool, Tentacruel (rare)

Altering Cave is the other half: it holds the Helix Fossil, Dome Fossil and Old Amber, plus a Moon Stone and Sun Stone — but the fossils only appear with "New Encounters" enabled. That is also where Mewtwo is, through the same cave.

Battle Frontier Reward Shop

The shop sells the four event tickets and a handful of items you cannot buy elsewhere in Hoenn: Eon Ticket, Mystic Ticket, Aurora Ticket, Old Sea Map, an Up-Grade, and the Macho Brace, which multiplies EV gain by five. The Frontier PokéMart next door stocks Safari Balls, Max Revives, Ethers and Elixirs, PP Ups, and — only when Reusable TMs is switched off — every TM in the game.

Trainer Hill

Trainer Hill hands out Luxury Balls and the XD/Colosseum berries — Salac, Ganlon, Petaya and Apicot — and pays money based on your finishing time, up to 1,000,000₽ if you clear it inside ten minutes. It also pays 20 Battle Points per win.

Notable held items

These are held by wild Pokémon and, with the Item Drops option on, are picked up automatically when you defeat them — no Thief required:
