Modern Emerald Evolution Items: Every Stone, Where to Get It, What It Evolves

Modern Emerald uses 16 different items to evolve Pokémon, and a few of them work by being held while you level up rather than being used — that is the part that catches people out.

Where to get them

Sun Stone and Moon Stone — Altering Cave, and both are sold at the Slateport Market (they replaced Hidden Power and Secret Power on the stock list).
Shards — Rock Smash drops, and held by specific wild Pokémon: Green (Wailmer, Wailord, Relicanth), Red (Corsola, Carvanha, Sharpedo), Yellow (Chinchou, Lanturn, rare), Blue (Tentacool, Tentacruel, rare). Altering Cave trades shards for stones.
Deep Sea Tooth and Deep Sea Scale — held by Clamperl (common) and Chinchou/Lanturn (common) respectively.
Heart Scale — Magikarp, Togepi and Feebas always. This is the most farmable evolution item in the game.
Up-Grade — the Battle Frontier Reward Shop, or held by wild Porygon.
Lucky Egg — Pelipper (rare), Happiny, Chansey. Evolves Happiny into Chansey.
Other held evolution items — Soothe Bell (Chimecho), Black Glasses (Mightyena), Miracle Seed (Shroomish), Charcoal (Torkoal), NeverMeltIce (Snorunt), Magnet (Elekid, Electabuzz), Silk Scarf (Happiny, rare).

The two that are not items in the usual sense

Feebas to Milotic is Beauty 170+, not an item. Beauty comes from PokeBlocks, and tamato berries are the reliable way to raise it. If you enabled "Easier Feebas" the hunt is much shorter.
Roselia to Roserade is Beauty 70+, a much lower bar.

Trade evolutions still exist

Nothing needs a Link Cable and nothing was converted to a stone: trade evolutions are still trade evolutions, and the hack gives you a Self-Trader in Lilycove so you can do them on a single save.
